Hermaphrodite ke?

Write when you feel like writing and you have the time. Macam sekarang. Berapa bulan entah tak update blog. Heck, who cares?

I was having a long weekend. 3 days of holidays and going to start new posting, the Medicine next week. So basically I have nothing to worry about this weekend. No case sheets, no SDLs, no study kaw2 for kelas lecturer garang or block tests. I am counting my hours left for Ramadhan kareem. Esok puasa!! yeay.

Few days ago I read about a case of a person who want to change his name in the IC to a female name. I kind of not really following the case deeply in the media. But today, he was found dead due to unstable angina and cardiogenic shock. He was buried as a male.

Based on what had I googled, he was born and raised as a male. Currently he is 26 years old. Two years ago he underwent a surgery in Thailand to change his gender and from that on, he lived as a female with a new name. Problems came when he wanted to register himself in the college as a female as he required a female IC. Jabatan Pendaftaran Negara rejected his request. So he brought the case to court and he lost. He was described to become depress after that and later aggravate his hypotension (low BP) and lead to his cardiogenic shock.

The reason why the court rejected his proposal was because of the 3 factors. The chromosome, the presence of male genitalia and the presence of male internal organs. Im not sure if he is actually a hermaphrodite or 'khunsa' in Malay. A true hermaphrodite when a person has both male and female genitalia. Then the hukum of khunsa will become different, like when he/she grows up, they will live as a gender that he/she most dominant.

I remembered when there was a newborn who had an ambigous genitalia. The doctors could not decide his/her gender. But since there was a male genitalia, so the parents were adviced to raise him as a boy while waiting for the karyotyping, hormonal and enzyme analysis result. My lecturer told us that you have to remember that actually it is not easy to become a male. Once there is the male feature, then he is a male. It could be a hypospadias since the newborn did not pee through the male urethra, instead there was a small lumen like a female urethra that pass the urine. There a few differential diagnosis that I do not remember. Sometimes I am so sleepy in the class.

Anyway this is a different case. We did not know if this is a hermaphrodite case. Or could be a psychiatry case, transexualism. Could be an endocrinal diseases. There a few more disorders I had came across along this few years of study. It does sound weird and rare. But they are there in the community. I have seen few rare syndromes that I thought I only see and read about them in the medical book. So I believe, there are few more people like this in this country who have problem with their gender identity.

I did know much about the law in Malaysia. Is it that easy to change the gender in IC if it was been misdiagnosed? Or should we actually refer this case to Mahkamah Syariah? Is there any law about this? I dont know. But one thing Im sure, there are hundreds of people like this who did not voice out their problem. Their confusion is blended with the perception of the community. What if suddenly they have to live as opposite gender after been living as a former gender for years. Orang mesti pandang slack. They just dont know the condition. What if the case if the mother really want them to stay as a male, but she actually a female? or vice versa? The mother may not understand this. Somebody needs to wake them up about the truth. Make people understand that they are people like this in this society.

Hey, Im not saying that I agree with homosexuality and related stuffs. That is different stories. Itu cerita pondan. Ini cerita betul dari sudut perubatan.
